VOLUNTEER CODE OF CONDUCT

I, understand as a condition of becoming and remaining a member of the SHEROES United, I have a responsibility to ensure that the Organization does the best work possible in pursuit of its goals and to adhere to the following expectations:  As part of my responsibilitiesas an Official representative of SHEROES United, I agree to:

General Expectations

Support the organization’s mission, purpose, goals, policies and programs, while knowing its strengths and needs.  Follow the bylaws, and uphold the board’s legal and charter responsibilities.Keep agreements to serve, including time commitments and completing assignments in a timely manner. Attend activities and events sponsored by the organization whenever possible.

I will support and encourage the DirectorsI am assigned and work in good faith with senior staff and other board members as partners toward achievement of our goals. I believe in the organizations mission and purpose, and I will act responsibly and prudently as its steward. I will also keep current with the website, social media and the blogs. I will also be well educated and can share with others clearly the SHEROES Mission.

Meetings

Prepare for and participate in Committee and Volunteer meetings, Town Meetings and the Annual Business Meeting including appropriate organizational activities as determined.  Keep active files and forms updated, and kept in secure location. This includes digital files. All files are to be kept in confidence and under board supervision. Maintain confidentiality of the sessions and executive sessions and not disclose any information that is designated as confidential or is disclosed under conditions in which a reasonable recipient would realize that the Confidential Information is confidential. I commit to making myself available as often as I can afford to do so without interrupting my daily life.

Avoiding Conflicts

Serve the organization as a whole and act in the best interests of the organization, avoid even the appearance of a conflict of interest and disclose any possible conflicts to the board in a timely manner.  Avoid asking special favors of the staff, without prior consultation with the CEO, Board Chair, or committee chairperson. I will avoid prejudiced judgments on the basis of information received from individuals and urge those with grievances to follow established policies and procedures through their supervisors.

Fiduciary Responsibility

At any time while in SHERO Attire or while representing SHEROES United, I will act in accordance of the organization. I adhere to absolutely, no gossiping, negative images on social media or lewd language. As a representative of SHEROES United, I will not consume any alcoholic beverages during SHEROES Events.As a SHERO I am committed to uplift and inspire fellow SHEROES and all individuals that I come into contact with.

Fundraising 

Assist the organization by implementing fundraising strategies through personal influence with others, i.e., corporations, foundations, and individuals. Participate actively in all fundraising special events and Thank You Follow ups, etc. If I do not fulfill these commitments to the organization, I will expect the board chair to call me and discuss my responsibilities.

SHEROES Commitment

The organization will provide a group of like-minded people that I will be working together towards same goals, an organization that vets out amazing individuals and works to provide Education, Resources and assists in the transformation to improve the quality of my life. Opportunities will be offered to me to discuss with the CEO and the Board Chairs the organization’s programs, goals, activities, and status; additionally, I can request such opportunities.  The organization will assist me to perform my duties by keeping me informed about issues in the area of interest, which we are working, and by offering me opportunities for professional development.

VOLUNTEER CONFIDENTIALITY/NON-DISCLOSURE AGREEMENT

This Agreement, and in consideration of volunteering for SHEROES United, the undersigned volunteer hereby agrees and acknowledges:

  1. That during the course of my volunteer services there may be disclosed to me certain information, materials, products and documents of the SHEROES United; said information, materials, processes, products and documents consisting but not necessarily limited to: (a) Technical information: Methods, processes, equipment, computer programs, log-in passwords, privileges and resources. (b) Business information: Customer lists, donors, employee data, and personal data; salary scales and rates; social security numbers; financial data and marketing, production, or merchandising systems or plans or anything originating from activities related to SHEROES United.
  2. I agree that I shall not during, or at any time after the completion of my volunteer services with SHEROES United, use for myself or others, or disclose or divulge to others including future volunteers, contractors or employees, any confidential information, or any other proprietary data of the SHEROES United in violation of this agreement.
  3. SHEROES United reserves the right to take disciplinary or legal action up to and including termination for violations of this agreement.
  4. That upon the completion of my volunteer services from SHEROES United: (a) I shall return to SHEROES United all documents and property, including but not necessarily limited to: computers, computer programs, equipment, manuals, correspondence, customer lists and personal data, reports, and all other materials and all copies thereof relating in any way to SHEROES United business, or in any way obtained by me during the course of volunteering. I further agree that I shall not retain copies, notes or materials of the foregoing. (b) SHEROES United may notify any future or prospective third party of the existence of this agreement, and shall be entitled to full injunctive relief for any breach. (c) This agreement shall be binding upon me and my personal representatives and successors in interest, and shall inure to the benefit of SHEROES United, its successors and assigns.

 

PHOTOGRAPH & VIDEO RELEASE AGREEMENT

 

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration.  I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.  I also understand that this material may be used in diverse educational settings within an unrestricted geographic area.

By signing this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in the public educational setting.There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

This release applies to photographic, audio or video recordings collected at any SHEROES United events or events that SHEROES United attends.

By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.

VOLUNTEER LIABILITY FORM AND WAIVER

This is a waiver and release of liability – please read carefully before signing.

 

The undersigned (“Volunteer”) wishes to perform work and provide services on a voluntary basis at no charge or cost to the ​SHEROES United​ a non-profit organization whose headquarters are located in Salt Lake City, UT.

SHEROES United has three key pillars.  1. Domestic Violence 2. Human Trafficking3.Women in Uniform /1st Responders.

Our work will be vetted to always stay in line with the pillars, so Volunteers will perform around those key issues and must be aligned with the SHEROES United Mission.

The exact nature of the work and services to be performed vary from project to project. The Volunteer’s activities may include, without limitation, the following risks and hazards: 1) the use of tools and other equipment, 2) lifting and moving objects and setting up programs, displays, events, use of space for workshops and classes  3) working in rugged terrain, back country locations and exposure to the elements, and potential out of country missions if selected. 4)  Working in large crowds 5) other risks listed in the project description. I am aware that these risks and other hazards are inherent in participation in these projects and hereby assume sole responsibility for all such risks and hazard. I have read and understand the project description and duties that will be expected of me related to the particular project in which I will be participating. I agree to conduct myself in a safe and courteous manner and to accept supervision from SHEROES United staff and other project leaders, Tribe Leaders, Committee Heads. Follow chain of command using proper channels. I understand that if I fail to do so permission for me to participate in any project may be revoked by SHEROES United.

In return for receiving permission from SHEROES United, its partners and associates to participate in this project, I agree to assume all risks of loss and injury that may arise out of my participation and I agree to waive any and all claims against SHEROES United and all parties described below.

I hereby release and agree to indemnify and hold harmless SHEROES United, its partners, associates, collaborating agencies and organizations involved in this project, the participants in this project, including other volunteers, and their respective agents, representatives, officers and employees, assigns and insurers, hereinafter referred to as the “released parties,” from any and all claims, causes of action, losses and damages (at any time) which I may have, or may claim to have, whether known or unknown, arising out of, or related in any way to my participation as a volunteer for SHEROES United. I also release and forever discharge SHEROES United from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with my activities with SHEROES United This release and agreement shall be binding upon me, my heirs, successors, assigns, administrators and executors.

I understand that, except as otherwise agreed to by SHEROES United in writing; SHEROES United does not carry or maintain health, medical or disability insurance coverage for any volunteer.

Volunteers are required to have their own medical insurance coverage.

I hereby acknowledge that I have read, understood and voluntarily agree to the foregoing waiver and release agreement and do not require further explanation. I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of Utah and that this Release shall be governed by and interpreted in accordance with the laws of Utah.
